Improved multiple-choice responses in CSV export

When exporting study responses as a CSV, multiple-choice responses now have numeric representations, providing an easier way to analyze the data.

• Multiple Choice Single-Select questions now have an additional column containing the index number of the selection. For example, if the user selected the 2nd choice, this cell will contain “2”.
• Multiple Choice Multi-Select questions have an additional column for the selection state of each choice presented to the user. “0” indicates no selection, and “1” shows that a selection was made.
